Milton Court to Hear Charges in Fatal Oakville Collision and Police Misconduct Cases

The Ontario Court of Justice in Milton will be the venue for two high-profile cases following separate investigations by the Halton Regional Police Service (HRPS), including a fatal pedestrian collision in Oakville and criminal charges against a civilian police employee.

 

On November 6, HRPS responded to a tragic motor vehicle collision on North Service Road West, west of Lindsay Drive in Oakville. A westbound Lexus SUV, driven by a 47-year-old Oakville man, left the roadway and struck a 39-year-old woman and her 8-year-old daughter. Both victims were transported to hospital and later transferred to trauma centres. Despite lifesaving efforts, the child succumbed to her injuries. The driver was arrested and charged with Dangerous Operation of a Conveyance Causing Death. The HRPS Collision Reconstruction Unit has taken carriage of the investigation.

 

In a separate incident, HRPS announced the arrest of Ken Mackenzie, 42, a civilian employee of the service, following a criminal investigation into alleged online fraud and deception. Mackenzie, a Guelph resident, is charged with Fraud Over $5,000 and Obtaining Property by Deception. Police allege the accused used social media accounts, including Instagram handle @stephy_rk, to pose as a female modeling scout and obtain intimate images from adult victims under false pretenses. Investigators believe there may be additional victims and are urging anyone with information to contact Detective Sergeant Todd Martin or Detective Jody Gregory at 905-825-4777, ext. 8967. Mackenzie is scheduled to appear in Milton court on Monday, December 1, 2025.

 

“These allegations do not reflect the values of our service,” said HRPS Chief Stephen Tanner. “We are committed to upholding public trust and hold all members—sworn and civilian alike—to the highest standards of professionalism and conduct.”

 

Meanwhile, Milton court also remains the jurisdiction for bail hearings related to an ongoing sexual assault investigation in Oakville. Calogero “Carlo” Triassi, 46, was arrested on October 22 and faces multiple charges stemming from incidents between 2020 and 2025. These include three counts of Sexual Assault, three counts of Sexual Interference, two counts of Invitation to Sexual Touching, and charges related to the creation and possession of child sexual abuse and exploitation material. Triassi was held in custody pending a bail hearing in Milton. HRPS investigators are concerned there may be additional victims and are encouraging anyone with relevant information to contact the Child Abuse and Sexual Assault Unit at 905-825-4777 ext. 8970.

 

In Georgetown, HRPS continues to investigate a late-night shooting that occurred on October 29 near Tenth Line and River Drive. Two victims later presented at hospital with serious but non-life-threatening injuries. The Major Crime Unit is seeking dash cam footage from drivers in the area between 10:00 p.m. and 11:00 p.m. that evening.

 

As these investigations unfold, Milton’s courthouse remains a focal point for justice proceedings across Halton Region, underscoring the community’s role in addressing serious criminal matters and supporting victims through the legal process.
